Why Does Skin Look Crepey After 40? What Actually Changes

Crepey skin — that fine, crinkled, tissue-paper texture that usually shows up first on the forearms, chest and under the eyes — happens because the supporting layer underneath the skin thins while the surface layer holds less water. It is mostly three things stacked together: a slow decline in dermal collagen, decades of accumulated sun exposure, and dryness. Only some of that is within your control, and the research is unusually clear about which part.

What is crepey skin, exactly?

Crepey skin is a change in texture, not a wrinkle. A wrinkle is a fold in one place, usually where a muscle has creased the same spot for years. Crepiness is a fine, all-over crinkling across a broader area, and it appears most on skin that is naturally thinner or has taken the most sun: the forearms, the backs of the hands, the décolletage, the eyelids.

That distinction matters, because the two respond to different things. Folds are largely structural. Crepiness has a significant hydration component — which is why it can look noticeably better within hours of moisturising and noticeably worse in dry air, in a way a true wrinkle does not.

What actually changes in skin after 40?

Three layers change at once, and each contributes something different to what you see in the mirror.

The dermal part is measurable, and has been for a long time. A 1975 study in the British Journal of Dermatology measured skin thickness and collagen density across ages and found both decline with age (PMID 1220811).

Hormonal change adds to it. A study in Obstetrics and Gynecology tracking postmenopausal women found skin collagen content, skin thickness and bone mass all declining at a similar rate of roughly 1–2% per year after menopause, tracking more closely with years since menopause than with age itself (PMID 3120067). That is a small study by modern standards, and it measured collagen and thickness rather than how skin looked — but it anchors why the change often feels like it accelerates rather than creeps.

How much is age, and how much is sun?

More of it is sun than most people expect. Dermatology research separates two distinct processes: intrinsic (chronological) ageing, which happens everywhere on the body, and extrinsic ageing, driven overwhelmingly by ultraviolet exposure, which happens only where light lands. They produce different changes in the dermis and are, in effect, two different problems sharing one appearance (Rittié & Fisher, Cold Spring Harbor Perspectives in Medicine, 2015; PMID 25561721).

The test you can run on yourself
Compare the skin on your inner upper arm — which has barely seen daylight in your lifetime — with the skin on your outer forearm. Same age, same person, same genetics. The difference between them is roughly the share that is sun, not time.

This is good news, in a backhanded way. The portion driven by time is not something anyone can undo. The portion driven by light is partly preventable, and preventing it has the best evidence in the entire field.

What does the evidence actually support?

Daily sunscreen — the strongest result there is

A randomised controlled trial in Nambour, Australia followed 903 adults under 55 for four and a half years, randomly assigning them to daily broad-spectrum sunscreen or discretionary use, with skin ageing graded by assessors blinded to group. The daily-sunscreen group showed no detectable increase in skin ageing over the 4.5 years, and 24% less skin ageing than the discretionary group (relative odds 0.76, 95% CI 0.59–0.98) (Hughes et al., Annals of Internal Medicine, 2013; PMID 23732711).

Two things belong alongside it, because they are what make it credible. The authors noted their power to detect moderate effects was modest, and some outcome data were missing. And it was publicly funded by Australia's National Health and Medical Research Council — not by a sunscreen manufacturer.

The same trial also found something that did not work

That study had a second arm: 30 mg of beta-carotene daily versus placebo. It found no overall effect on skin ageing. A supplement widely sold at the time for exactly this purpose, tested properly in the same trial, did nothing measurable. We mention it because a source you only quote when it agrees with you is not a source.

Hydration — modest, real, and fast

Because a meaningful part of crepiness is the surface layer holding less water, humectants and emollients change how it looks relatively quickly. That is a genuine effect on appearance, and also a temporary one — it works while you keep doing it. Anyone telling you a moisturiser rebuilt your dermis is describing something else.

What to expect, honestly

  • Prevention beats correction, by a wide margin. The sunscreen evidence is about slowing what has not happened yet.
  • Surface texture responds within days. Structural change does not respond on that timescale, to anything sold over a counter.
  • The forearms and chest lag behind the face, because most people have spent decades treating one and ignoring the other.
  • If it changed suddenly, or came with other symptoms, that is a doctor's question, not a skincare one.

Frequently asked questions

What causes crepey skin?
A combination of declining dermal collagen, fragmented elastin from accumulated ultraviolet exposure, and a surface layer that holds less water than it used to. Sun exposure accounts for more of the visible difference than chronological age alone.

Is crepey skin the same as wrinkles?
No. Wrinkles are folds in specific places, usually from repeated muscle movement. Crepiness is a fine all-over crinkling of texture across a broader area, and it has a much larger hydration component.

Can crepey skin be reversed?
The hydration component improves quickly and visibly, but only while you keep treating it. The structural component — collagen density and elastin damage — is not something a topical product reverses. Prevention has far better evidence than correction.

Does sunscreen actually help with skin ageing?
Yes, and it has the strongest evidence in the field. In a 4.5-year randomised trial, the daily-sunscreen group showed no detectable increase in skin ageing and 24% less ageing than the discretionary-use group (PMID 23732711).

Why is it worse on my arms and chest than my face?
Those areas typically receive decades of sun exposure with far less protection and care than the face, and the skin on the chest and eyelids is naturally thinner to begin with.
