Why the dose is the story Twenty milligrams, against 2,700 mg of glucosamine-chondroitin in the head-to-head. The same category, two orders of magnitude apart. That ratio is the entire reason this fits in a gummy instead of a horse pill — and it is what the judging panel singled out.
How it is understood to work Q-Actin® is an iminosugar — a sugar ring in which a nitrogen atom replaces the oxygen. As described in the award citation, it acts on tumour necrosis factor alpha, an inflammatory cytokine whose overproduction raises the metalloproteinases associated with cartilage breakdown, and is understood to bind the CD44 hyaluronic acid receptor.†
Where most joint formulas stop Glucosamine, chondroitin, curcuminoids, collagen and eggshell membrane supply the building blocks for repair. The trade press puts it plainly: they do little to address the inflammation that often causes the damage in the first place. This is the other half of the problem.

What the authors concluded Improved sleep quality and finger dexterity. Dexterity on the Nine-Hole Peg Test: significant in the Q-Actin group at twelve weeks (p = 0.011) and between weeks six and twelve (p = 0.007), while the placebo group did not change at any timepoint. Sleep on two lines of evidence — elevated melatonin derivatives in urine, confirmed by LC-MS/MS fragmentation, alongside self-reported improvement on the Pittsburgh Sleep Quality Index.
What it did not show Grip strength improved in both groups and did not separate — the authors attribute that to a placebo or learning effect from repeated testing. The total PSQI sleep score did not reach significance; the component that moved was subjective sleep quality at six weeks (p = 0.020).

The authors' own limitations, in their words: the sample was relatively small, self-reported sleep measures such as the PSQI may be subject to bias, and future studies should incorporate objective monitoring such as actigraphy or polysomnography. We print those beside the result, because a finding without its limits isn't evidence — it's advertising.
